Care Transitions Program:

Under the leadership of Dr. Eric Coleman, the aim of the Care Transitions Program is to improve the quality and safety of care hand-offs (e.g., transfer from one care facility or to another, or from hospital to home) for persons with complex care needs.

See, in particular, the tools developed by Dr. Coleman with funding from the John A. Hartford Foundation and the Robert Woods Johnson Foundation to help the elderly and/or their caregivers manage transitions from one care setting to another while minimizing the high potential for medical mishap.

Speak Up:  Help Prevent Errors In Your Care - Brochures

In March 2002, The Joint Commission, together with the Centers for Medicare and Medicaid Services (CMS), launched a national campaign to urge patients to take a role in preventing health care errors by becoming active, involved and informed participants on the health care team. 

The Joint Commission has made various brochures available for your personal use, to download and/or to print. These documents help hospitals and patients safeguard transitions and discharges from care settings.

Senior Mentor Programs:

Funded by the John A. Hartford Foundation, the Robert Woods Johnson Foundation, the Arthur Vining Davis Foundation, the American Association of Medical Colleges, and other foundations, Senior Mentor programs are designed to help incoming medical students develop a sensitivity and understanding of the complex needs of elderly patients in order to ameliorate the serious problems associated with a growing aging population faced with retiring specialists in geriatric medicine. Donald W. Reynolds Foundation awards $20 million for geriatrics training.
